Our palliative support offers free, non-medical, short-term support to maintain the best quality of life possible for individuals whose health needs mean they are required to stay home.

The service is funded by the Black Country Integrated Care Board to provide support to those who have palliative or end of life needs within all WV postcodes.

The support we provide includes:

  • Sitting with your loved one so that they are not alone, allowing you to maintain work or family commitments
  • Accompanying your loved one to appointments. For example hospital appointments, GP appointments, chiropodists appointments etc
  • Social calls in person or on the phone if you are on your own for a long period of time
  • Emotional support
  • Advocacy

Referrals to our service can only be made by healthcare professionals – including GPs, district nurses, social and care workers. To refer someone to our palliative support service, please download and fill in the referral form and send it to Accord.group@nhs.net.
